Confidential Feedback Form.
It is very helpful for us to receive feedback about the service we have offered. It would therefore be appreciated if you would complete the feedback form and return it to the above address. Stating your counsellor’s name is optional. If you do state their name we will forward it on to them.

Formal Complaint: Each therapist belongs to a professional body that deals with formal complaints.
If any therapist has acted unethically we want to know and can direct you as to where to make the complaint.
Formal complaints are not usually a pleasant processess for either party.
If it is more of misunderstanding, than misconduct we can try and help you both resolve it before it escaletes to a complaints procedure.
